DSD NEX/DEX Resource Library

Each DSD supplier is required to complete a DSD Survey Form. The purpose of the DSD Survey Form is to obtain information about your delivery technology capabilities as well as the appropriate individuals to contact to coordinate the conversion process.

1. Regardless of technology capabilities, all suppliers are requested to complete section A: General Information.
2. Suppliers indicating NEX capability should complete section B
3. Suppliers indicating DEX capability should complete section C
4. Fax the completed DSD Survey Form to (401) 652-0377
